Acknowledgement of Documents

Note:

  • All clients must fill up and submit the Health Declaration form on-site when availing services at the Consulate.

When persons executing or making the declarations in the document (e.g., power of attorney, affidavits, certifications, contracts) appear in person and sign the document before a Consular Officer at the Consulate General, the document will be notarized by the Consular officer in the form of an AcknowledgmentJurat or Certification, as the case may be. The Consular Officer in this case performs the functions of a Notary Public. Hence, individuals personally appearing before a Consular Officer are no longer required to have their documents notarized by a Notary Public before submitting the same for notarization.

Requirements :

For Acknowledgment (Special Powers of Attorney, General Powers of Attorney, Deeds of Sale of Property, Deeds of Donation, etc.)

  1. Original and one photocopy of the document;
  2. Two (2) Original government issued IDs with two (2) photocopies each
  3. The fee of CAN $ 36.25 (Cash or Money Order payable to the Philippine Consulate General)

Applicant must provide the following:

  1. The number of original documents (including all attachments) needed for consularization and one (1) photocopy of each different document (including all attachments) for the Consulate’s copy.
  2. 2 (two) Original government-issued IDs with two (2) photocopies for each document
  3. Fee of CAN $ 36.25 per document

For Jurat (documents such as Affidavits of Support and Consent, Parental Travel Permits, Complaints, Affidavits of Illegitimacy, Applications for Marriage License and Oaths of Professionals)

  1. Original and one (1) photocopy of the document;
  2. Two (2) Original government issued IDs with two (2) photocopies each
  3. The fee of CAN $ 36.25 (Cash or Money Order payable to the Philippine Consulate General)

Applicant must provide the following:

  1. The number of original documents (including all attachments) needed for consularization and one (1) photocopy of each different document (including all attachments) for the Consulate’s copy.
  2. 2 (two) Original government-issued IDs with two (2) photocopies for each document
  3. Fee of CAN $ 36.25 per document

For Certifications (document issued by the Consulate certifying that the applicant personally appeared at the Consulate – for SSS/GSIS purposes)

  1. The original and two (2) photocopies each of the following:
    1. Passport;
    2. Two (2) Original government issued IDs with two (2) photocopies each,
  2. ​A fee of CAN$ 36.25 (Cash or Money Order payable to the Philippine Consulate General) is charged per document authenticated.

Applicant must provide the following:

  1. The number of original documents (including all attachments) needed for consularization and one (1) photocopy of each different document (including all attachments) for the Consulate’s copy.
  2. 2 (two) Original government-issued IDs with two (2) photocopies for each document
  3. Fee of CAN $ 36.25 per document

 

Processing Times

Regular Processing:

Three (3) working days excluding mail delivery-time. Because of the COVID 19 situation, pick- up of documents is currently not permitted. For the release of their consularized documents, clients must submit a self-addressed Canada Post registered mail or ExpressPost envelope.

The Consulate is NOT RESPONSIBLE for delays or loss of documents mailed, or any other uncontrollable circumstances surrounding acknowledgment or authentication of your documents.

Expedite Processing :

On-the-day release of consularized documents may be requested with an additional expedite fee of CAD$14.50 per document (not pages).
