​​​​Notice to Passport Applicants

Personal appearance is required for all passport applicants.

Applicants are encouraged to book an appointment online for their convenience.


  • Legal instruments such as Affidavit/s for Legitimation or Acknowledgment and Admission of Paternity or Use of Surname of Father may be filed directly with City Civil Registry Office (CCRO) in Manila after securing notarization from the Philippine Consulate General.

  • To facilitate the processing by CCRO Manila of the legal instrument, applicants are advised to send their notarized legal instruments to their relative or designated representative in the Philippines who will file, on their behalf, and will perform the following tasks:

  1. To have the legal instruments verified by the Authentication Division of the Department of Foreign Affairs, as part of the requirement of the CCRO Manila, if there be any doubt on their authenticity, and
  2. To pay CCRO Manila the prescribed fees for the registration of the legal document.

  • After processing at CCRO Manila, the legal instrument will be forwarded to the Philippine Statistics Authority (PSA) for registration.

  • 9:00 am to 5:00 pm (Monday to Friday)
  • 9:00 am to 6:00 pm (every last Friday of each month)​

except on Philippine & Canadian holidays.
