Personal appearance is required for all passport applicants.

Applicants are encouraged to book an appointment online for their convenience.

MINOR APPLICANT (Below 18 years old)
Pursuant to instructions from the Department of Foreign Affairs, the Philippine Consulate General in Vancouver wishes to inform the public that effective 01 January 2018, the following requirements for passport applications for MINOR applicants will be implemented:

FOR NEW APPLICATION OF MINOR APPLICANT:

Core Requirements

  • Accomplished Application Form
  • Personal Appearance of minor applicant and either parent or authorized adult companion
  • Birth Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO)
  • Passport or Valid Government issued ID of either parent
  • Original and photocopy of one of the following: Applicant’s valid Permanent Resident Card/Work Permit / Student Visa, Dual Citizenship Certificate (Order of Approval/Identification Certificate), Record of Landing (if within 1 year from date of arrival),  or latest Tourist Visa to Canada, whichever is applicable.  Status Verification from IRCC (formerly CIC) is required if Permanent Resident Card has been expired for more than two years.


Supporting Documents
In addition to the core requirements, the following SUPPORTING DOCUMENTS could be further required in the following cases:

In case Minor Applicant has No PSA Birth Certificate/Report of Birth yet:

If born in the Philippines: Certified True Copy of Local Civil Registrar Birth Certificate authenticated by PSA
If born abroad: original copy of Report of Birth or first indorsement from Consular Records Division

In case Minor Applicant is not accompanied by the parent/s during the application process:

  • Applicant must submit a notarized Special Power of Attorney executed by the parent/s designating the minor’s companion to assist in the application process.
  • Passport or Valid Government issued ID of authorized adult companion


In case Minor Applicant is an illegitimate child in custody of the Mother:

  • Personal Appearance of minor applicant and mother or authorized adult companion
  • Birth Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO)
  • Passport or Valid Government issued ID of mother or authorized Adult Companion (click here for the List of Acceptable IDs for Passport Application)
  • If applicant is not accompanied by mother during the application process, applicant must submit a notarized Special Power of Attorney executed by the mother designating the minor’s companion to assist in the application process.
  • If applicant is not traveling with Mother, applicant must submit notarized Affidavit of Support and Consent executed by the mother.

If the Minor Applicant is an Illegitimate child and mother is deceased / absent and father is unknown:

  • Personal Appearance of minor applicant and adult guardian
  • Birth Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO)
  • Valid Passport or valid government ID* of adult guardian
  • Notarized Affidavit of Guardianship

If the Minor Applicant is undergoing or has undergone the process of domestic adoption:

  • Personal Appearance of minor applicant and adoptive parent/s
  • Birth Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO)
  • Valid passport or valid government issued ID of adoptive parent/s
  • Court Decree of Adoption
  • If applicant is not accompanied by adoptive parent/s during the application process, applicant must submit a notarized Special Power of Attorney executed by the adoptive parent/s designating the minor’s companion to assist in the application process.
  • If applicant is not traveling with adoptive parent/s, applicant must submit a notarized Affidavit of Support and Consent executed by the adoptive parent/s.


If the Minor Applicant is undergoing or has undergone the process of foreign adoption / Inter Country Adoption Board (ICAB) adoption process:

  • Personal Appearance of minor applicant and ICAB representative
  • PSA Birth Certificate/PSA Certificate of Foundling
  • Endorsement from ICAB
  • Certificate of Child Available for Adoption/Deed of Voluntary Commitment
  • Placement Authority issued by ICAB
  • Certificate for Issuance of Passport issued by ICAB
  • Clearance for Inter-Country Adoption
  • Child Study Report
  • DSWD clearance


If the Minor Applicant is a foundling NOT for adoption:

  • Personal Appearance of minor applicant and adult guardian
  • PSA Certificate of Foundling
  • Passport or Valid Government issued ID* of adult guardian
  • DSWD clearance
  • Notarized Affidavit of guardianship


PASSPORT RELEASE  AND DELIVERY

New passport will be available approximately 8 weeks* after date of application.

A self-addressed prepaid return envelope (with tracking number) PER APPLICANT must be provided upon submission of passport application.  Applicant may secure said prepaid envelope from Canada Post (Xpresspost envelope) or from a courier service of their choice.

The Consulate, however, does NOT assume responsibility for passports/documents that may be lost in the mail.

*Barring any circumstance beyond the Consulate’s control, passport applications will be processed and released by above-stated period.

MODE OF PAYMENT – Passport Fee: CAD 81.00

Applicants who apply for passport processing at the Philippine Consulate General’s office in Vancouver may pay in: (1) Cash, or (2) Bank draft or postal money order payable to the Philippine Consulate General.  Please note that debit or credit card transactions are not available.

Applicants in an Outreach Program must pay by MONEY ORDER or BANK DRAFT ONLY, payable to the Philippine Consulate General.  One (1) money order or bank draft may be accepted for applications of a group of applicants, e.g. a family of 4 applicants may pay with a bank draft or money order of CAD 324.
